ABOUT

Form Health opened in 2022 and began with a simple vision: to create a space where patients feel genuinely cared for--from the science behind the treatment to the smile at the front desk.

For founder Dr. Mecca Fayad, that dedication to service started early. Raised in Lac La Biche, Alberta, she spent her childhood in the back of her father’s Kentucky Fried Chicken, where she learned the importance of hard work, hospitality, and community. Those lessons inspired Form Health’s foundation: clinical excellence, exceptional service, and a beautiful, welcoming atmosphere.

Today, the clinic unites a multidisciplinary team of healthcare providers who share a common goal—to help people live better, healthier lives. Rooted in the heart of downtown Edmonton, Form Health continues to give back through local initiatives, events, and collaborations that keep community at its core.

Dr. Mecca Fayad

Chiropractor

Dr. Mecca Fayad earned her Bachelor of Science in Biology from the University of Alberta before graduating Cum Laude from Palmer College of Chiropractic West in San Jose, California. In 2025, she received the Clinical Excellence Award for her dedication to community projects and patient-centered care within the chiropractic profession.

Dr. Fayad is passionate about delivering evidence-based, individualized care for patients of all ages—from pediatrics and pregnancy to active professionals and athletes. She holds certifications in Active Release Technique (ART), Webster Technique, RockTape, and Instrument-Assisted Myofascial Release.

When she’s not in the clinic, Dr. Fayad can often be found travelling with her husband, walking through Edmonton’s River Valley, checking out the latest fitness class or exploring local restaurants and bakeries. She also loves visiting her hometown of Lac La Biche, where her journey began.

Dr. Naomi Velado-Tsegaye

Chiropractor

Dr. Naomi was born and raised in Edmonton, Alberta. She completed her undergraduate degree in Immunology at the University of Alberta before moving to San Jose, California, where she earned her Doctor of Chiropractic degree from Palmer West.

Dr. Naomi takes a patient-centered and evidence-based approach to care, focusing on finding and treating the root cause of pain or injury. She combines techniques to help her patients move and feel their best. She has a special interest in working with pregnant patients, children, and youth athletes-helping them stay active, supported, and confident in their recovery. 

She has worked with professional athletes and our local Golden Bears. She was recently awarded the "Up and Coming Chiropractor" award in 2024 for her work with underserved communities and fundraising events. She’s dedicated to making chiropractic care more accessible and approachable for everyone, from growing families to young athletes.

Sylvia Rullo

Kinesiologist & (almost) Chiropractor

Dr. Sylvia is a graduated Doctor of Chiropractic from Palmer West in San Jose California, with a Cum Laude honour and a clinical excellence nominee. While she awaits her Canadian Chiropractic License, she uses her graduate knowledge and Bachelor of Kinesiology degree from the University of Alberta to provide rehab, exercise prescription, cupping, scraping, and taping!

She is beyond grateful to be a part of a career that allows her to help people feel better in their everyday life or favourite activities. She values a collaborative approach with patients, finding care that is the most attainable and effective, so that goals can be met! Dr. Sylvia is passionate about holistic care and the many working parts that go into your overall health and wellbeing. With that, she continues to educate herself and her patients, empowering them along their wellness journey and providing them the necessary tools for self-care. 

Outside the clinic, Dr. Sylvia enjoys long walks through the city trails, picking up new hobbies, travelling, and spending time with family and friends.

Vincent Sy

Physiotherapist

Vincent has practiced in Alberta, British Columbia, and Virginia over the past nine years. Vincent completed his Bachelor’s of Human Kinetics at the University of British Columbia (UBC) in 2010 and continued his studies there for his Master’s of Physiotherapy, completed in 2012. Since graduating, he has pivoted away from a purely structural approach, embracing the complexity and challenges faced when working within a more holistic person-centred model. He has competed and regularly participated in volleyball, soccer, and enjoys kayaking and going for walks with his whoodle, Charlie.

From a personal and professional outlook, Vincent understands that rehab, training, and performance all exist on a continuum. He recognizes that establishing skilled movements for performance and in daily life can help build confidence which in turn allow for long term sustainable change. He actively works with his clients, building resiliency and self-efficacy through a combination of manual therapy, exercise, and education. Vincent has a special interest in working with thoracic outlet syndrome, runners, and knee (patellofemoral) pain. While working with all clients, he is a movement optimist, believing in the variability and the durability of the body and the inherent ability to adapt and improve given the right stimulus.

Michael Olila

Massage Therapist

Michael is a Registered Massage Therapist with CRMTA. He graduated from Makami College in 2018 with 3000 hours of clinical experience and was awarded with a Certificate of Distinction in Advanced Clinical Massage Therapy.

Since then, Michael has helped clients with repetitive stress injuries, postural dysfunction, chronic pain, migraines and more. His multifaceted style ranges from Swedish relaxation to therapeutic deep tissue techniques. He offers Dynamic Cupping, Assisted Stretching and RAPID-NFR in his treatments to promote healthy movement in the joints and tissues.

Michael is motivated to help his community live pain free and to achieve a better quality of life through massage therapy. He strongly believes in the bodies natural ability to heal given the proper care, treatment plan and knowledge. During his free time, Michael enjoys getting outside exploring the river valley, practicing yoga, travelling, shopping the local farmers market and spending time with his friends and family.

Destiny Neufeld  

Massage Therapist

Destiny is a Registered Massage Therapist who graduated from the West Coast college of Massage Therapy.

Her goal is to provide personalized care to assist each patient in reaching optimal health. To provide long term resolution, she looks for the root cause of your problem while addressing symptoms for immediate relief.

She uses a variety of techniques and tailors each treatment to your specific individual needs. Her current methodology contains a mix of Swedish techniques, Myofascial Release, Deep Tissue modalities, and Neuromuscular Facilitation techniques. She has a passion for injury rehabilitation and prevention and experience working with a broad range of patients, including athletes, pre- and post-natal, motor vehicle accidents, and those with overuse injuries or postural dysfunctions. Her approach uses soft tissue manipulations during treatment, combined with stretching and strengthening recommendations as home care, to achieve optimal therapeutic effect .

Chris Saskiw  

Massage Therapist

Chris is a registered massage therapist that graduated in 2021 with honours from Makami College in Edmonton, Alberta.

"Massage has so many benefits for physical, mental, emotional and spiritual wellbeing. Pain relief from sore muscles, increased flexibility, relaxation, mental clarity, improved circulation, sports recovery, and injury prevention. Chris wants to help others achieve optimal health and exceed their goals. He is passionate towards providing a sense of well-being to his clients and has spent hours on treating clients with acute, sub-acute and chronic conditions. He is skilled in Swedish, Sports, Trigger Point Therapy, Deep tissue, Kinetic techniques, nerve flossing, and myofascial release.

In his spare time Chris enjoys visits with family and friends, travelling, playing ice and in-line hockey, and pretty much any outdoor activity/adventure.

Charmaine Gentles   

Massage Therapist

I am Charmaine Gentles a graduate of Makami College, 3000 hours Diploma in Massage Therapy. I have practicing for over 7 years in Edmonton and Jamaica. I am enthusiastic about my craft, particularly about my clients’ well-being, which includes their physical and emotional well-being.

My inspiration to become a massage therapist stems from my deep passion for people and a fervent desire to make a positive impact on their lives. My areas of specialization include a variety of massage techniques, tailored to meet the unique needs of my clients. These may include Swedish Massage, Deep Tissue Massage: Hot Stone Massage and Thai Massage, TMJ, Scalp and Feet Massage therapy.

Lorena Alvarez de la Cuadra, Dr.Ac

Acupuncturist

Lorena is a Registered Acupuncturist and MacEwan University graduate with distinction. Her practice is grounded in evidence-based acupuncture, focusing on chronic and acute pain, orthopedic support, nervous system regulation, and restorative therapies.

Along with traditional acupuncture techniques, she integrates Facial Rejuvenation Acupuncture,  TMJ protocols, Dao Needling Orthopedic acupuncture, and Dolphin Neurostim microcurrent therapy for vagal nerve and pain modulation. Her favourite modality is sliding cupping, valued for its grounding and tension-releasing effects.

A former anatomy and physiology tutor for MacEwan students, Lorena brings strong anatomical knowledge and clinical precision to her treatments. Originally trained as a Doctor of Veterinary Medicine in Mexico City, she combines a deep understanding of the body with a calm presence and restorative approach to support lasting healing and balance.

Lesley Smail  

Office Manager

You’ll often spot Lesley hovering around the coffee machine, crafting the perfect peppermint tea or indulging in a delicious latte. She’s recently celebrated a huge milestone — 12 years in her career as a Medical Office Administrator!

Lesley has been working alongside Dr. Fayad since 2015, keeping things running smoothly and bringing her signature mix of organization and good humour to the office. When she’s not keeping Dr. Fayad & the team on track, you’ll find her chatting with the many patients she’s gotten to know — and befriend — over the years.

If you ever have a question, don’t hesitate to ask Lesley. Her well of knowledge is sometimes vast... and sometimes mysterious, but one thing’s for sure — she’ll always point you in the right direction!

In her spare time, you’ll find Lesley tending to her garden, strolling through the scenic river valley, or diving into her latest passion — self-publishing romance novels.

Angela Juson            

Receptionist

Angela is one of the friendly faces welcoming you to Form Health. She is working towards her undergraduate degree in Psychology, where she spends most of her time studying and volunteering at her university. She is dedicated to creating a warm and welcoming space for every client, since she believes that a supportive environment is a major part of personal wellness. Outside of work and studies, she enjoys going to pilates, solving sudoku puzzles, and hanging out with her sibling and friends.

Cassandra Devlin       

Receptionist

Born and raised in Edmonton, Cassandra graduated from MacEwan University in 2023 with a Bachelor of Arts, majoring in Psychology. She will soon be pursuing her Master’s in Counselling with the goal of becoming a psychologist. With a strong passion for psychology, Cassandra believes that caring for your physical health plays an important role in supporting your mental well-being. She is here to welcome you with a warm smile and offer you a beverage, helping make your visit to Form Health comfortable and inviting. Cassandra also contributes her creativity to our social media presence! 

In her spare time, she loves watching movies, spending time with friends, and cuddling her cat, Sunny.

Jamie Chow            

Receptionist

Is currently enrolled at the University of Alberta, taking a bachelors in science. She is always curious about the many different healthcare industries, which led her to join the Form Health team.

Jamie’s goal is to gain experience and make many new memories, but is always willing to lend a helping hand. Whether that’s making coffee, filling out intake forms, or helping to book your next appointment! 

In her spare time she enjoys volunteering, playing ice hockey and is hoping to get her motorcycle license soon!
